Job Description

CarePartners is seeking a compassionate and clinically skilled Registered Nurse Inpatient Hospice GIP to join our Healthcare & Medical Services team supporting our partnered inpatient hospice facilities. As an integral member of our coordinated care network, you will provide high‑quality General Inpatient (GIP) hospice nursing care to patients with complex, acute needs, while collaborating closely with physicians, case managers, social workers, and other interdisciplinary team members to ensure comfort, dignity, and symptom management at the end of life. In this role, you will perform comprehensive assessments, develop and implement individualized plans of care, administer medications and treatments, monitor and promptly address changes in patient condition, and provide expert guidance and emotional support to patients and their families. You will document accurately and thoroughly in the electronic health record, communicate effectively across disciplines and care settings, and help ensure smooth transitions of care that align with hospice philosophy and patient goals. Aligned with CarePartners’ mission to improve community health, you will advocate for patient-centered care, support best practices in pain and symptom management, and help educate patients, caregivers, and facility staff about hospice services and end‑of‑life care. This position requires strong critical thinking, excellent clinical judgment in an inpatient or hospital setting, and the ability to prioritize and respond rapidly in a dynamic environment. The ideal candidate brings solid experience in acute care or inpatient hospice, demonstrates compassion and cultural sensitivity, and works collaboratively to promote a safe, respectful, and healing environment.
